Admission to the Nursing Specialty Advanced Certificate Program in any of the three courses (wound, ostomy, foot 和 nail) 需要 申请人必须:

  • A registered nurse (RN) with a nursing-major Baccalaureate Degree or higher (BSN, MSN, DNP或PhD)

-- Or --

  • A registered nurse (RN) with a Bachelor's Degree in another field, 和
  • One year of RN clinical nursing experience following RN licensure, 和 
  • Current clinical nursing experience or completion of a nurse refresher course or a clinical performance examination within five years prior to application

Apply early, as it takes time to request 和 receive official transcripts from academic 机构.

The application process has three main steps:

      1. Create an Application in Cal State Apply
      2. 程序专用文档
      3. 官方成绩单

Step 1: Create an application in Cal State Apply

Cal State Apply is the application portal for the entire California State University (CSU) system, for multiple start terms.

  • From the main page, scroll down toward the bottom 和 select the term in which you wish to enroll (via a dropdown menu next to a red Apply button).
  • During the process of creating your account, you will be asked to indicate your degree 目标/类型
    • --> select "second bachelor’s degree 和 beyond 和 certificate."
  • After being directed to the Find Program or Add Program page, type "San Jose" in the "Filters" search bar to narrow your search (example image below). 这将带来 up the "San Jose Extension" section, in which you will find the “伤口 和 Ostomy Nursing Specialty Advanced Certificate” (this is the program through which you will apply for any of the specialty certification 项目: 伤口护理, 造口术护理, 和/or 脚 & 指甲保健).

The following additional application documents for the School of Nursing must be uploaded to the Cal State Apply website during the application process:

Step 3: 官方成绩单 submission (after submitting initial application via CalStateApply)

After you have submitted your application, you will receive an email from the Graduate Admission Office confirming receipt of your application. This email will include step-by-step instructions for setting up your 菠菜网lol正规平台 account 和 monitoring the status of your application.

If accepted into 一个 or more of the three Care 项目, some additional steps will 需要.

All three of the specialty certification Care 项目 require a $100.00应用程序 fee (in addition to the Cal State Apply submission fee), which must be paid via Transact before the beginning of the program.

背景调查

Clinical agencies require that all students have cleared a criminal background check 和 a negative urine drug screen prior to starting clinical. 注册护士学生负责 for obtaining the required background check 和 urine drug screen upon notification 的验收. Information on how to obtain background check 和 urine drug screen will be described on the notification 的验收.

If you plan on taking your wound 和/or practicum in Santa Cruz, Bakersfield, or at your own employment facility, please check with us first before obtaining background check, urine drug screen, 和 other tests.
